Job Summary
Caseworker coordinates mental health and supportive services across healthcare providers, social service agencies, schools, housing programs, and community partners within a community-based behavioral health model. Establishes measurable goals aligned with client strengths and recovery objectives, tracks client progress, documents services for clinical necessity and billing compliance, and supports clients in navigating complex systems to achieve greater independence and stability. Requires flexible scheduling and Illinois standards as an MHP or QMHP; degree in Social Work, Psychology, Counseling, or related field; experience in community-based case management and with homeless/high-risk populations and individuals with serious mental illness (SMI) is preferred.
Required Qualifications
- You Must meet Illinois standards as a Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP).
- Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related behavioral health field.
