Job Summary
Develop Individual Support Plans for assigned caseloads and coordinate services to assist individuals in carrying out their plans. Maintain consumer progress notes, complete PUNS processes, and conduct phone contacts with consumers, families, physicians, or others regarding needs and services. Act as an advocate for clients in school, community, or other needs areas while counseling families, individuals, and providers. Interview clients to obtain social histories, facilitate meetings with providers, and complete requests for evaluations or referrals. Work with individuals requiring crisis intervention and enter client data into HCSIS for record maintenance. Travel frequently to homes and providers, with some evening work for appointments or meetings. Requires six months of Human Services Case Manager experience or a bachelor's degree with related social science credits and professional casework experience.
Required Qualifications
- Six months of experience as a Human Services Case Manager
- A bachelor's degree with a social welfare major
- A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social services agency
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences
- Meritorious service: defined as the absence of any discipline above the level of written reprimand during the 12-months preceding the closing date of the posting
