Job Summary
Assist in providing direct casework services to patients and families within the hospice program of care. Utilize community resources and knowledge of local, county, state, and federal assistance programs to support patient care operations. Work with minimal supervision while maintaining proficiency in company software programs. Requires a Bachelor's Degree in Social Work or related field, two years of healthcare experience in community resources utilization, and reliable transportation. Candidates must be able to cope with the stress of experiencing repeated loss. Equal Opportunity Employer.
Required Qualifications

- Extensive knowledge of community resources within the specific community in which the hospice is located

- Capacity to work with minimal supervision
- Ability to become proficient in company software programs
- Bachelor's Degree in Social Work or a related Social Science field from an accredited school
