Job Summary
Assist in providing direct casework services to patients and families within the hospice program of care. Utilize community resources, including local, county, state, and federal assistance programs, to support patient care operations. Maintain proficiency in company software programs while operating with minimal supervision. Requires a Bachelor's Degree in Social Work or related field, two years of healthcare experience in community resources utilization, and reliable transportation with adequate insurance.
Required Qualifications

- Ability to become proficient in company software programs
- Bachelor's Degree in Social Work or a related Social Science field from an accredited school
