Job Summary
Perform comprehensive assessments for individuals seeking long-term care services and public funding or Home and Community-Based Services (HCBS). Conduct face-to-face consumer visits to determine the appropriate Level of Care, collaborate with family members, physicians, hospitals, and agencies, and ensure assessments meet 10-business-day standards. Utilize WellSky and PDA guidelines, complete required forms for Dom Care/Personal Care Home residents, maintain accurate records in HIPAA-compliant systems, and provide testimony related to level-of-care determinations. Travel to assessments is required; position involves data entry and familiarity with aging program policies and community services for adults 18+. Requires ability to communicate effectively, organize work, maintain confidentiality, and support consumers through the assessment and appeal processes. Required Qualifications
- Bachelor's Degree (or equivalent combination including 12 semester hours in sociology, social welfare, psychology, gerontology, or related social sciences) or 1 year of assessment experience with a social welfare major;
- 2 years of casework experience including 1 year performing assessments of clients' functional ability to determine need for institutional or community-based services;
- Ability to review medical/healthcare needs to determine appropriate levels of care;
- Knowledge of aging program policies and procedures;
- Experience using WellSky and PDA guidelines for assessments;
- Ability to maintain records and documentation in HIPAA-compliant manner;
- Pre-employment clearances including background check, child abuse clearance, drug screening;
- Possession or acquisition of required assessor credentialing (through Aging Well, PA) within four months;
- Valid driver's license and ability to transport to home visits and appointments.
