Job Summary
Conduct home visits to map family living environments and needs, then lead casework from simple to complex issues including safety concerns. Determine appropriate care or support aligning with the Youth Act, while managing the entire process through clear communication and analytical judgment. Support families in developing support plans, monitor progress, and actively participate in integrated meetings to ensure optimal outcomes. Maintain a broad network of partners and coach colleagues to foster knowledge sharing and professional growth within the team.
Required Qualifications
- Relevante hbo-opleiding zoals Social Work, Pedagogiek, Orthopedagogiek
- Kennis van en/of ervaring met relevante wet- en regelgeving, waaronder de (Verlengde) Jeugdwet
- Relevante ervaring met de doelgroep jeugd en gezin
- SKJ-registratie of de mogelijkheid deze te behalen
- Opgedane ervaring bij een gemeente
- Relevante ervaring als Consulent Jeugd
