Job Summary
Conduct independent intakes and coordinate care for clients with serious psychiatric conditions in their own living environments. Lead treatment and recovery journeys by setting restorative goals with clients, family, and multidisciplinary partners while monitoring progress and adapting support based on changing situations. Contribute to the development and implementation of the ART methodology, actively participating in care quality improvements and team initiatives. Maintain clear communication with external collaborators and manage administrative tasks with precision. Requires a BIG-registered nursing or social work degree, valid driver's license, and 28 hours weekly availability. Based at GGz Breburg in Etten-Leur within a collaborative, recovery-focused team.
Required Qualifications
- Een afgeronde hbo opleiding verpleegkunde met BIG registratie of een hbo opleiding Social Work
- Je werkt herstelgericht en hebt oog voor de wensen, mogelijkheden en eigen regie van de cliënt
- Je schakelt moeiteloos tussen verschillende hulpvragen en weet daarin professioneel en zelfstandig te handelen
- Je organiseert je werk goed, houdt overzicht en coördineert je afspraken zelfstandig
- Je communiceert helder, werkt graag outreachend en bouwt gemakkelijk een vertrouwensrelatie op met cliënten en samenwerkingspartners
- Je bent flexibel, stressbestendig, administratief zorgvuldig en brengt daarnaast een gezonde dosis humor mee
- Je bent in het bezit van een rijbewijs en beschikt over een auto
- Je bent 28 uur per week beschikbaar
- Een Verklaring Omtrent het Gedrag (VOG) en een referentiecheck maken onderdeel uit van onze selectieprocedure
