Job Summary
Case Worker responsible for trauma-informed, person-centred case management for individuals and families experiencing homelessness or at risk of homelessness. Responsibilities include developing and reviewing client case plans per SHS guidelines, assisting access to safe housing, addressing psychosocial needs, conducting home visits, performing assertive outreach, building partnerships with government agencies and community organisations, conducting risk assessments, maintaining accurate records and reporting, supervising volunteers, participating in team meetings, and upholding safety and inclusive practices. Requires knowledge of local support services, strong communication, and the ability to work both independently and within a team. Minimum Certificate IV in Community Services or related field; NSW Driver Licence preferred.
Required Qualifications
- Minimum Certificate IV in Community Services, Welfare, or a related discipline, or currently undertaking tertiary studies in a relevant field.
