Case Worker - Supported Independent Living (SIL)

Job Summary

Facilitate development of individual case plans for adults with developmental disabilities at risk of or involved in the criminal justice system. Connect clients to community resources, educate them on life skills, and assess risk regarding community safety. Liaise with team members and community organizations, while documenting case notes and maintaining relationships with family supports. Regularly travel in your own vehicle between facilities and locations. This role supports people with mental health needs or addictions, potentially involving one-on-one crisis intervention for individuals with potential for verbal or physical aggression. The program operates 8am to 10pm, seven days a week, with a rotation of 12-hour shifts.

Required Qualifications

  • Post-secondary education from an accredited institution
  • 1-3 years' work experience dealing with people with developmental disabilities, acquired brain injuries, addictions services, and/or fetal alcohol spectrum
  • 1-3 years' experience working with people who have the potential to become verbally or physically aggressive, using de-escalation strategies to mitigate risk
  • Standard First Aid/CPR with AED - Level C
  • Valid G Class Driver's License
  • access to a reliable vehicle with appropriate insurance
  • clear Vulnerable Sector Screening Police Check

Desired Qualifications

  • Case management experience
  • proficiency in both official languages of Canada
