Job Summary
Coordinate inpatient and post-acute services by conducting assessments, developing treatment and discharge plans, and managing aftercare coordination. Collaborate with patients, families, physicians, and community providers to identify needs, secure resources, and facilitate referrals while documenting all activities in the TIER system. Provide guidance to help individuals navigate illness impacts and plan for successful recovery. Requires a BSW with two years of case management experience, preferably in discharge planning, and strong skills in healthcare systems and care coordination.
