Job Summary
Provide clinical case management and direct support services to children and families in a diverse, multicultural environment. Collaborate with schools, DCF, therapists, attorneys, court personnel, and community partners to develop individualized service plans, coordinate resources for safety, stability, and permanency, advocate for children’s educational, emotional, behavioral, and social needs, and maintain accurate documentation and case records. Build strong relationships with children, families, and community stakeholders while delivering trauma-informed, strengths-based practice.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred
