Job Summary
Provides comprehensive, structured case management services for emergency shelter residents. Conducts resident intake and an initial needs assessment, develops comprehensive program plans/goals, and evaluates residents’ progress through one-on-one sessions and diagnostic tools. Advocates for community resources to resolve underlying causes of homelessness, refers residents to counseling resources as needed, and maintains accurate case records and logs (HCIS). Collaborates with external case management professionals to meet holistic resident needs, and works in a shelter facility with attention to attendance and clear communication. Requires an Associate degree in social work, human services, or related field and at least one year of direct case management experience, or an equivalent combination of training and experience.
Required Qualifications
- Associate degree in social work, human services, or related field from an accredited college or university
- One year experience providing direct case management social services including accessing residents’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources
- Any equivalent combination of training and experience providing the required knowledge, skills, and abilities
