Job Summary
As a Case Worker at Horizon, you will conduct intake screenings to assess behavioral health needs, connect individuals to mental health and substance use treatment services, and facilitate communication between providers, courts, and state agencies. This role involves maintaining documentation, addressing treatment barriers, and educating participants on court processes. Required qualifications include a bachelor's degree in a relevant field and one year of professional experience in mental health or related services.
Required Qualifications
- Bachelor’s degree in social work, counseling, psychology, or a related behavioral health or human services field required
- One year of professional experience in mental health, criminal justice, substance use, or related human services required
Desired Qualifications
- Strong knowledge of local behavioral health and criminal justice systems
- Background in case management and care coordination
Additional Requirements
- Eligible for a sign-on bonus of up to $1,500 for commitment to work within the Navigator Grant for at least one year
