Job Summary
Coordinate Medi-Cal eligibility screenings and activate Justice-Involved aid codes in the state reentry portal to support the successful reintegration of CalAIM-eligible individuals. Gather clinical and logistical information, monitor release dates, document consents, and ensure timely EHR documentation while supporting continuity-of-care handoffs under the clinical oversight of the RN Discharge Planner. Work closely with managed care plans, in-reach providers, and community partners to coordinate service delivery and ensure compliance with CalAIM requirements. This role helps individuals transition back into the community with access to needed services, supporting a mission to provide quality care with compassion and innovation.
Required Qualifications
- Strong communication, documentation, and coordination skills
- Working knowledge of Medi-Cal processes
- Working knowledge of CalAIM policy requirements
- CPR certification
- Any other certifications (such as First Aid) required by contract
Desired Qualifications
- Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university
- Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness
