Job Summary
Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization. Conduct comprehensive assessments and develop individualized service plans. Coordinate multidisciplinary interventions tailored to each family's unique needs and build meaningful relationships over an average case duration of 12 months. Collaborate with internal and external providers to ensure safety, stability, and well-being. Citywide travel is required for up to 14 cases with four visits per family monthly. This role supports at-risk children and families and those with developmental disabilities through quality community-based services.
Required Qualifications
- BSW or bachelor's degree in human services REQUIRED
- Some knowledge of the child welfare system
- Citywide travel required- 14 cases up to 4 visits with each family per month
Desired Qualifications
- Fluency in English and Spanish a Plus
