Job Summary
Conduct monthly contacts with foster children and biological parents to assess safety, well-being, and progress toward permanency. Coordinate visitation plans ensuring weekly parent-child visits and re-assess them based on the child's development. Schedule and participate in Family Team Conferences while maintaining contemporaneous CNNX records within 48 hours of contact. Submit FASPs and Permanency Hearing Reports, reporting and testifying at Family Court proceedings including permanency hearings and TPR. Maintain regular contact with service providers and monitor all child services including medical, clinical, and education needs. Complete mandated trainings and perform activities to further the agency's mission. Work 35 hours weekly Monday through Friday, 9:00 AM to 5:00 PM, with some evening and weekend hours required.
Required Qualifications
- MSW or related degree
- Bachelor degree with two years of relevant experience
- Valid Driver's License
- Available car for field visits
- Ability to use a computer
- Ability to learn software programs
- Ability to speak, read and write fluently in the English language
- Ability to write in the English language using correct spelling, grammar, punctuation, etc.
- Ability to frequently lift 0-25 lbs.
- Ability to occasionally lift 25-50 lbs.
- Ability to rarely lift over 50 lbs.
- Ability to frequently push/pull 0-25 lbs. of force
- Ability to occasionally push/pull 26-45 lbs. force
- Ability to rarely push/pull over 45 lbs.
- Ability to sit 50-60% of the working day
- Ability to stand 10-20% of the working day
- Ability to walk 10-20% of the working day
- Ability to occasionally stoop
- Ability to occasionally kneel
- Ability to occasionally crouch
- Ability to frequently work in an office environment
- Ability to tolerate moderate noise levels
- Ability to tolerate increased noise levels due to yelling or screaming
