Job Summary
Case Planner (PREIS) supports AFGs mission by delivering quality case management services to youth-focused participants in the EmpowHer program. Responsibilities include performing community outreach to recruit and enroll participants, meeting with participants and parents to guide enrollment, conducting two initial meetings to assess needs and create an action plan, doing follow-ups and providing additional resources, advocating for girls on school and family issues, participating in case conferences, maintaining participant records and data entry, ensuring program quality, scheduling, and budgeting, compiling statistics for reports, and developing relationships with schools and agencies. Demonstrated abilities include strong organizational skills, prioritization, communication, diplomacy, decision-making, conflict resolution, confidentiality, crisis management, and ability to handle job stress. Required qualifications include an associate or bachelor’s degree in relevant fields, 3-5 years of related experience, valid driver’s license and reliable transportation, basic computer skills, and English-Spanish bilingual ability preferred.
Required Qualifications
- Requires an associate or bachelors degree in social work, counseling, psychology, sociology or criminal justice
- Minimum of 3-5 years of related experience
- Must possess a current driver's license, a good driving record and reliable transportation
- Requires basic computer software and Internet skills
- Bilingual in English and Spanish is preferred
