Job Summary
Case Planner in CAMBA School-Based Early Support: conducts family outreach and advocacy, develops individualized support plans with families, serves as client advocate with schools and health providers, maintains confidentiality, manages a caseload of 12 clients, documents progress (FASP reports, databases), participates in case conferences, and coordinates with ACS and community partners to support child safety and school partnership efforts. Requires a BA/BS/BSW and at least two years of experience, strong writing and organizational skills, and bilingual ability (English plus Spanish or Haitian Creole preferred). Role includes monthly Home visits and school-based services typically lasting 6-12 months; compensation is $53k-$55k annually, and the position is full-time (35 hours/week).
Required Qualifications
- BachelorBSBSW or equivalent
- Minimum of two years of experience working with families and children in a community setting
- Strong writing, interpersonal, and organizational skills
- Proficiency in Microsoft Office Suite
- Bi-lingual English and Spanish or Haitian Creole (preferred)
