Job Summary

Case Monitor for NYC Department of Health and Mental Hygiene's Office of Assisted Outpatient Treatment. Responsible for monitoring court orders and treatment plans for individuals eligible under Kendra’s Law; verify weekly contact with Care Coordinators and treatment teams to ensure service providers fulfill duties; document changes to treatment plans and maintain consumer records; assist in developing and reviewing treatment plans; facilitate involuntary transport documentation and provide psycho-social summaries for hospital evaluation; participate in case conferences to discuss eligibility and progress under orders; collaborate with providers and hospitals to coordinate services; require strong knowledge of community mental health resources, interpersonal and organizational skills, and ability to interface with multiple service providers.

Required Qualifications

  • Graduation from an accredited graduate school of social work evidenced by a master's degree or certificate
  • Within 18 months of appointment, obtain a certificate as Certified Social Worker (CSW) issued by the New York State Department of Education
  • Experience in public health, medical or psychiatric social work; supervisory/administrative/consultative capacity desired
