Job Summary
Collaborate with the clinical team and medical providers to deliver patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients with complex health needs. Supplement the existing staff by filling in for absences and vacations, focusing on complex discharge arrangements including SNF, LTC, and hospice. Promote patient and family wellness and access to appropriate hospital and community resources. Requires a Master's in Social Work with an LMSW license, 3-5 years of experience, and the ability to follow verbal instructions and communicate effectively.
Required Qualifications
- Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline
- Master's in Social Work
- LMSW
- 3-5 years of experience
- MD Social Work License per level of education
