Job Summary
Coordinate discharge planning with the treating psychiatrist and team to ensure proper linkage with family or support systems within 24 hours. Conduct crisis assessments of home environments, determine risk levels, and facilitate duty-to-warn protocols when indicated. Perform initial pre-admission screenings under supervision and communicate with courts, DCS, DHS, schools, and other agencies regarding admission, progress, and post-discharge follow-up. Complete treatment plans, progress reports, and patient releases while maintaining accurate medical records. Schedule and facilitate aftercare appointments, group sessions, and individual psycho-education to ensure compliance. Collaborate with therapists, families, and business office representatives to resolve benefit issues and finalize discharge plans.
Required Qualifications
- BSW or BA/BS in closely related discipline
- Non-related degree with five (5) years relevant experience
- Experience in direct patient care in the mental health field
- Knowledge of chemical dependency
- Knowledge of child and adolescent development
- Knowledge of chronic mental illness
- Knowledge of DSM IV diagnosis
- Knowledge of psychiatric inpatient treatment
- Crisis stabilization skills
- Valid Tennessee driver's license Class D
- State mandated minimum insurance coverage
- Driving record meeting Covenant Health minimum standards
- Local travel required
Desired Qualifications
- Knowledge of psychiatric inpatient treatment is beneficial
