Job Summary
Coordinate discharge planning with the treating psychiatrist and team to ensure timely linkage with family or support systems. Conduct crisis assessments of home environments, determine risk levels, and facilitate duty to warn protocols when indicated. Perform initial pre-admission screenings under supervision and communicate with courts, DCS, DHS, schools, and other agencies regarding admission, progress, and post-discharge follow-up. Complete treatment plans, progress reports, and patient records while maintaining updated releases. Schedule aftercare appointments, facilitate family conferences, and escort patients to outside agencies as needed. Maintain communication with outpatient providers and follow up on discharge outcomes as directed by the Program Therapist.
Required Qualifications
- BSW or BA/BS in closely related discipline
- Non-related degree with five (5) years relevant experience
- Experience in direct patient care in the mental health field
- Knowledge of chemical dependency
- Knowledge of child and adolescent development
- Knowledge of chronic mental illness
- Knowledge of DSM IV diagnosis
- Knowledge of psychiatric inpatient treatment
- Crisis stabilization skills
- CPR
- First Aid
- OSHA Bloodborne Pathogens standards
- Valid Tennessee driver's license Class D
- State mandated minimum insurance coverage
- Driving record meeting Covenant Health minimum standards
- Local travel
