Job Summary

The Case Management Coordinator is responsible for engaging individuals in a supportive manner, screening for co-occurring disorders, and assessing the needs of clients, developing strength-based treatment plans. This role includes advocating for services, completing necessary documentation for community-based services, and collaborating with other mental health agencies. Candidates must have relevant experience in evaluating and treating minors and their families, along with specific licensure in the State of Michigan.

Required Qualifications

  • Possess a Bachelor's degree from an accredited school and have three years supervised experience in the examination, evaluation, and treatment of minors and their families
  • Possess a Bachelor's degree from an accredited school and be an LLBSW or LBSW with the State of Michigan and one year of experience in the examination, evaluation, and treatment of minors and their families
  • Be a Master’s prepared LLMSW, LMSW, LLPC, LPC, TLLP, or LLP with the State of Michigan with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families
  • Must be a CMHP in accordance with Medicaid Provider Manual Guidelines
