Job Summary
Case Manager position within the Supervised Independent Living Program (SILP), a senior direct care role supplying engagement, counseling, and support to residents aged 18-20 to stabilize and develop independent living skills, with goals for discharge back to the community. Responsibilities include managing a caseload, evaluating independent living skills, advocating with other service providers, linking residents to community-based treatment, collaborative service planning, interpreting policies, potential group facilitation, supporting medication-management independence, accompanying residents to appointments, and participating in intake/discharge processes, staff meetings, and trainings. Requires a Bachelor’s degree (Master’s preferred) and experience with adolescents/young adults facing trauma and mental health challenges; strong communication, documentation, and the ability to travel between locations; familiarity with Windows, ACS records, and telehealth. Required Qualifications

- Familiarity with Connections (ACS record) preferred
- Familiarity with telehealth
