Job Summary
Conduct case management for vulnerable youth by interviewing participants, observing behavior, and meeting with caregivers to identify problems and risks. Develop and implement family-centred intervention plans, then facilitate group or individual sessions using active listening and conflict resolution techniques to resolve issues. Provide skill-building in communication, resiliency, and systems navigation while evaluating progress and reporting to supervisors. Liaise with community providers, schools, and natural supports, accompanying families to appointments as needed. Maintain accurate records and statistics for program oversight.
Required Qualifications
- Diploma in a related human / social service field
- Valid First Aid Certificate
- Class 5 driver's licence
- Clean drivers abstract
- Successful completion of Agency screening including a Criminal Record Check
- Legally entitled to work in Canada
