Job Summary

Develop goals and service plans for clients experiencing homelessness, including single women, domestic violence victims, and those with behavioral health disorders. Provide direct trauma-informed case management services involving crisis intervention, outreach, ongoing assessments, and resource utilization to transition clients to safe housing. Participate in clinical case reviews, weekly staffing meetings, and conflict resolution using de-escalation skills. Maintain case records, statistical data, and research projects while utilizing agency vehicles for transportation. Work on-site in an office environment, adhering to a regular schedule, punctuality, and on-call duties as needed.

Required Qualifications

  • A degree in the field of social services
  • Two years' experience working in the social services field
  • Any combination of education, experience, and training which provides the following knowledge, skills and abilities
  • Qualify for and pass Utah Department of Human Services Case Management Certification within 60 days of being hired
  • GED or High School Diploma required for Utah Case Management Certification
  • Familiarity with Trauma Informed Care
  • Familiarity with Motivational Interviewing
  • Familiarity with Housing First philosophies
  • Willingness to incorporate Trauma Informed Care, Motivational Interviewing and Housing First philosophies in daily work
  • Ability to uphold professional boundaries
  • Ability to uphold confidentiality regulations
  • Ability to interact in a professional manner with a diverse workforce, clients, and the public
  • Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities
  • Willingness to reach out for supervision for major situations
  • Willingness to accept supervision and direction
  • Computer literate with proficiency in Microsoft Office
  • Become CPR and First Aid Certified
  • At least 21 years of age
  • Possess a current driver's license
  • Have a good driving record
  • Be insurable on the agency liability policy
  • Must be able to pass Utah-DHS – Office of Licensing background screening
  • Must be able to pass pre-employment drug screen
  • Ability to lift and carry 25 lbs
  • Ability to walk up and down stairs
  • Ability to move around the facility, interacting with clients for extended periods
