Job Summary
Conduct initial assessments to determine program eligibility and create individual service plans with residents to address obstacles. Provide ongoing case management support including budgeting assistance, referrals to outside providers, and accompaniment to appointments. Maintain case records, complete data entry in databases, and prepare weekly reviews and quarterly reports. Perform housing unit inspections using HUD guidelines and document crisis interventions as required. Ensure a safe work environment through daily area inspections and adherence to health and safety practices.
Required Qualifications
- High school diploma or equivalent with required experience
- 3 years of experience working in the human services field and/or Case Management Experience
- Experience with low income chronically homeless, youth, substance use, serious mental illness, DV survivors, veterans, and/or HIV/Aids populations needed
- Must have knowledge of entitlement benefits programs and resources available for vulnerable populations
- Must have good communication and interpersonal skills and the ability to work with diverse groups
- Computer literacy and database experience in excel and access helpful
- Must have current driver's license and reliable vehicle for travel
- Must be able to regularly talk or hear
- Must be able to frequently stand; walk; under hand to finger, handle, or feel; and reach with hands and arms
- Must be able to bend, kneel, stoop, crouch, crawl, or climb
- Must be able to frequently lift up to 20 pounds
Desired Qualifications
- Associate's degree or higher in human services related field (social work, psychology, or mental health counseling)
- CASAC, or other relevant certifications highly recommended
