Job Summary

The Case Manager assesses clients’ needs, plans a course of action, links clients to services, and monitors the progress of clients and residents in their program. The Case Manager ensures that clients receive appropriate information and referrals to obtain and maintain housing. Duties include intake and assessment to orient clients, developing individual service plans, weekly meetings to support goals, coordinating services with Housing Specialist and external partners, advocating for clients around entitlements, crisis intervention and supportive counseling, documenting activities, completing housing applications, meeting monthly placement quotas, facilitating workshops and escorting clients, attending meetings and trainings, and flexible scheduling including evenings and weekends.

Required Qualifications

  • Bachelor's Degree preferred
  • Proof of Education is Required
  • At least 1 year of case management experience with homeless, mentally ill, substance abuse, and/or low-income populations
  • Bilingual a plus
  • Strong relationship-building skills
  • Ability to work independently
  • Excellent organizational and time management skills
  • Strong PC skills including Word and Excel
  • CARES experience preferred
