Case Manager

Job Summary
Collaborate with counselors to provide comprehensive case management services for patients diagnosed with substance use disorders, mental health conditions, or co-occurring disorders. Educate and refer clients to community agencies for food, shelter, and employment resources while utilizing Electronic Health Record systems for data entry and tracking. Conduct client screenings, facilitate support groups, and act as advocates to improve access to necessary services. Complete documentation on provided forms and participate in clinical staffing decisions regarding patient strengths and resource needs. Assist staff in a collaborative manner to promote cooperation and portray a professional image to the community. Bachelor's degree in human services required; CAP preferred.

- Ability to exercise good judgement and discretion.
- Strong Microsoft Office Suite experience
- Ability to complete work tasks within scheduled work hours.
- Excellent verbal skills and written communication skills
- Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services
- Bachelor's degree in human services field
- A minimum of 40 hours per week
- Completion of job duties may require working before and after normal working hours
- Regular attendance
- Being able to work as many hours as is necessary to complete job tasks
