Job Summary
Behavioral Health Case Manager in Palm Beach Shores, FL provides comprehensive case management services to individuals with mental health and/or substance use disorders from admission to discharge. The role conducts comprehensive assessments, develops individualized care plans, coordinates therapy, medication management, and support services, connects clients with community resources, participates in treatment planning meetings, advocates for client rights, provides crisis intervention, maintains accurate documentation, and collaborates with an interdisciplinary team to support recovery and adherence to regulatory standards.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling, or a related field
- Minimum of 3 years of experience in behavioral health case management
- Current and valid Florida license
- Experience working with diverse populations
- Proficiency in electronic health records (EHR) systems
- Current CPR and First Aid Certification
- Successful completion of Pre-Employment Requirements including a criminal background clearance, drug testing, and health screening
