Job Summary
Provide client-centred case management, advocacy, and support to young people while developing and reviewing personalised support plans. Deliver youth-friendly guidance, facilitate educational programs and health workshops, and coordinate access to community resources. Maintain accurate case records and respond to crisis situations through client assessments within a multidisciplinary team. Conduct community outreach across the Gold Coast to build relationships with stakeholders and partner organisations. This part-time role involves 30 hours per week. We offer salary packaging up to $15,900 annually plus wellbeing support, and seek candidates with a degree in social welfare and relevant case management experience.
Required Qualifications
- A completed degree in relevant social welfare matters
- Understanding of family relationship issues involving adolescents
- Strong case management principles
- Experience with intake, advice, advocacy and referrals
- Commitment to social justice principles, confidentiality and privacy
- Experience in establishing and maintaining agency network relationships within the human service environment
- Proof of COVID-19 vaccination status
- Relevant probity checks
- Current Driver's Licence
- Willingness to travel for work
