CASE MANAGER - WEEKEND

Job Summary
Coordinate discharge planning and transitions of care for a designated patient population, assessing options, negotiating resources, and facilitating referrals to prevent readmissions. Manage utilization reviews by initiating assessments within 24 hours of admission, monitoring treatment responses, and conducting continued stay reviews against payor contracts. Collaborate with the Denial Management Specialist to resolve adverse determinations, maintain appeal databases, and ensure timely correspondence to all payors. Work with the interdisciplinary team to expedite appropriate discharge, manage alternate levels of care, and track avoidable days in the Midas System.
Required Qualifications
- Three years clinical experience in healthcare
- recent case management experience
- proficient documentation skills
- ability to work well with diverse and challenging populations
- ability to maintain appropriate professional boundaries
- ability to remain calm during crisis situations
- Regular, consistent, on-site, and timely attendance
- Ability to perform the Essential Functions on the Physical Conditions chart
- Ability to perform the Essential Functions on the Working Conditions chart
