Job Summary
Provide client assessments and orientations; develop individualized case plans; connect clients to housing and community resources; assist with benefits, identification documents, and healthcare access; enter accurate documentation in HMIS; monitor progress and adjust plans; collaborate with internal teams and partner agencies; transport clients to appointments as needed.
Required Qualifications
- Associate degree in a related field, or equivalent human services experience
