Job Summary
Case Manager for The Glen at Webster Campus overseeing quality of care, case management, and overall service provided to residents with a dementia-specific focus. Responsibilities include developing and implementing Individualized Service Plans of care for Assisted Living residents, ensuring documentation complies with NYSDOH guidelines, performing formal and informal evaluations, ongoing needs assessments, and coordinating medical, nursing, psychosocial, and other services. Maintains Resident Electronic Health Records, participates in on-call rotations, and contributes to audit and quality assurance for NYSDOH compliance. Collaborates with the care team and marketing, maintains community relationships, and engages in resident and staff satisfaction activities to promote person-centered care and quality of life for residents.
Required Qualifications
- Bachelor's Degree in Social Work, Human Services, or Nursing OR LPN/RN with related experience
- 3 years of related experience (for Bachelor's path) OR 5 years of related experience (for Associate path)
- Must attend all mandatory in-service training and continuing education requirements
- Understanding of geriatric resident care and ability to provide services per NYSDOH guidelines
- Experience with developing and reviewing Individualized Service Plans of care for Assisted Living residents
- Ability to coordinate multiple services (medical, nursing, psychosocial) and collaborate with care teams
- Proficiency in maintaining documentation per NYSDOH guidelines
- Experience with Resident Electronic Health Records and regulatory documentation
- On-Call rotation involvement for resident emergencies
- Quality assurance and compliance responsibilities with NYSDOH
- Strong communication with residents, families, and staff
- Knowledge of dementia-specific care and person-centered approaches
- Ability to collaborate with administration, marketing, and community partners
- Bachelor's degree in Social Work, Human Services or Nursing, LPN/RN, plus 3 years of related experience
- Associate degree in Social Work, Human Services or Nursing, plus 5 years of related experience
