Job Summary
Lead Care Manager (Case Manager) within the Enhanced Care Management team, based in Orange County, CA. Hybrid in-field and remote role traveling up to 5 days/week; requires reside-within-20-miles of the assigned territory. Responsibilities include assessing member needs across physical/mental health, SUD, palliative memory care and social supports; developing client care plans; coordinating services via office, telehealth, or field-based visits; connecting clients to community resources; advocating with health professionals; and ensuring timely documentation in EMR/CRM. Utilizes Motivational Interviewing, Harm Reduction, and Trauma-Informed Care. Strong communication, organization, and computer skills; bilingual English/Spanish strongly preferred. Requires valid driver’s license, clean driving record, and willingness to travel within the Orange County area. Vaccination and background checks may be required. The role supports pacing data and reporting through monthly program compliance activities and is designed to improve wellness, independence, resilience, and member empowerment. Required Qualifications
- 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
