Job Summary
Lead Care Manager (Remote) coordinates and oversees care management for individuals with complex needs, serving as the primary client contact and liaison among physicians, specialists, social services, and family caregivers. Responsibilities include assessing needs across physical/mental health, SUD, oral and palliative care, memory and trauma-informed care, developing and linking care plans and SMART goals, facilitating access to services via office, telehealth, or field visits, advocating with healthcare professionals, documenting outcomes in EMR/CRM, arranging transportation, and ensuring program compliance. Requires 2+ years in care management or related roles, proficiency with Google Workspace and MS Office, ability to work Pacific Time, and bilingual English/Spanish preferred.
Required Qualifications
- 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends
- Working knowledge of government and community resources related to social determinants of health
- Clean driving record, valid driver's license, and reliable transportation
- Excellent oral and written communication skills
- Positive interpersonal skills required
- Must have general computer skills and a working knowledge of Google Workspace, MS Office and the internet
- Bilingual (English/Spanish) preferred
