Job Summary
Maintain a caseload of Veteran clients by developing housing plans and budgets, conducting assessments, and coordinating services with VA Medical Centers and community agencies. Assist households in applying for housing and subsidy applications, including Veterans Administration, Social Security, and other state/federal benefits. Help identify barriers to permanent housing, complete housing inspections for prospect units, and provide education on tenant rights. Connect Veterans to resources to maintain or obtain permanent housing while assisting in increasing household income for financial independence. Participate in individual supervision, team meetings, internal and external program audits, and trainings. Maintain regular communication with Veteran households and community partners, ensuring all services are documented and records are completed. Work hybrid with two days in the office and three days in the local service area, traveling throughout the South Shore and Cape Cod to meet clients in person.
Required Qualifications
- Ability to travel throughout the South Shore and Cape Cod to meet with Veterans in person
- Minimum of 2 years related experience in a case management role
Desired Qualifications
- BA/AA degree
- Knowledge of veteran issues, homelessness, and state & Federal resources
- Computer proficiency in electronic health records, Microsoft Word and Outlook
