Collaborate with healthcare providers and community organizations to ensure comprehensive care, serving as a communication liaison between families and the treatment team. Maintain accurate documentation and case records in compliance with regulatory standards. Participate in treatment planning meetings to communicate patient and family information regarding placement and aftercare details.
Required Qualifications
- Bachelor's Degree in Psychology or related field
