Job Summary
Assess members' health and safety needs to identify strengths, interests, and preferences, then develop a comprehensive Member Care Plan. Coordinate with a Field Care Nurse to provide services in the least restrictive environment following the plan. Conduct face-to-face social assessments upon enrollment and at minimum every six months, typically at the member's residence, while performing quarterly in-person visits and maintaining monthly phone contact. Arrange support services for social integration, community resources, employment, and housing needs, and ensure cost-effective service delivery. Evaluate risk factors and provide education to members, maintaining accurate documentation including case notes, service authorizations, and MCP updates. Work Monday through Friday, 8:00 am – 4:30 pm CST, with up to 40% travel required and a valid driver's license needed.
Required Qualifications
- A Bachelor's degree in human services or a related field
- At least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Reside within 45 mins or less of the assigned coverage area
- Case Management experience
- Experience with electronic case note documentation
- Knowledge of community health and social service agencies and additional community resources

- Must be able to support members in Viroqua

- Must be able to work in Viroqua, WI and surrounding area
- Must be able to work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
- Must be able to meet a download speed of 25 Mbps and an upload speed of 10 Mbps
- Must be able to be screened for TB
