Job Summary
PATH is seeking a Case Manager for the Site Based Housing team in the San Jose office. The role focuses on assisting clients experiencing homelessness to access and maintain housing through individualized planning, coordinating services, and leveraging community resources. Responsibilities include intake and needs assessment to develop an ISP addressing housing goals and income, providing referrals and support, completing program assessments (e.g., VI-SPDAT), maintaining documentation and confidentiality (HIPAA), building therapeutic relationships, delivering crisis intervention, applying motivational interviewing and trauma-informed care, and maintaining up-to-date knowledge of community resources to provide wrap-around services. The position emphasizes helping clients navigate housing and health needs across housing stages, with requirements including driving and on-site work in California, and a non-remote work arrangement.
