Job Summary
Coordinate and deliver direct case management services to 22 families, assisting with housing searches, financial barriers, and legal issues. Transport clients to appointments and develop service linkages with external providers while monitoring service plans and reporting data into HMIS/ETO systems. Facilitate workshops and community meetings, maintain building security, and document all activities in shift logs and staff communication logs. Provide Community Support Program services and become a Certified Application Counselor to offer enrollment assistance. Work collaboratively within the team to implement aftercare plans and advocate for clients with government agencies and landlords.
Required Qualifications
- Bachelor degree with 1-2 years' experience
- High school diploma with 3 or more years of client advocacy experience
- Relevant experience in housing search, housing stabilization and retention, case management, SUD services, employment training and job search services
- Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors
- At least 25 years old
- Valid Massachusetts driver's license with a safety driving points of 00, 99, 01, 02, or 03
- Knowledge of family shelter services or residential services
- Understanding of harm reduction philosophy
- Openness to harm reduction philosophy
- Ability to work independently and cooperatively as a member of a professional team
- Strong organizational, written, and verbal communication skills
- Strong computer skills, including Google office suites and Internet use
- Demonstrated ability to work successfully in multicultural settings with diverse client groups, including people with mental health concerns and substance use disorder
- Must be CPR certified
- Complete TB screening
- Three strong work-related professional references; at least 2 from manager or supervisor
