Job Summary
The primary purpose is to be a member of the patient’s care team to plan, develop and coordinate patient care in accordance with applicable standards, ensuring the highest quality of care; responsibilities include meeting with administration, medical and nursing staff, coordinating with social and community agencies for smooth discharge transitions, ensuring thorough documentation, assisting with admission and discharge processes, and contributing to care plan meetings with professional guidance.
Required Qualifications
- Minimum one year case management experience (preferred)
- Ability to make independent decisions
- Leadership ability
- Confidential information handling
- HIPAA compliance knowledge
- Ability to plan, organize, develop and interpret department programs/goals
- Pre-employment background check/drug screen
- Active Fingerprint card
