Job Summary
Case Manager for Army National Guard/Department of Defense health-care operations in Central Indiana. Responsible for facilitating case management initiatives and deployment operations, coordinating with Medical Records to ensure timely processing, conducting intake services, identifying medically non-deployable members, creating and tracking referrals in e-case, maintaining clinical notes, liaison work with MRNCOs and Commanders, ensuring medical documentation is uploaded to HRR/MedPROS, and coordinating with DHA-GL/TRICARE network providers for pre-authorization and continuity of care. Requires thorough documentation in eCase, MEDCHART, eMMPS and related systems, and administrative support across SRP and medical readiness events. Strong organizational, written and oral communication skills and ability to manage priorities are essential.
