Job Summary
Conduct active case management for a caseload of 22-25 clients, meeting weekly individually to assess barriers to housing and develop personalized housing plans. Transport clients to appointments and sites as needed while maintaining comprehensive documentation in HMIS within one business day of service changes. Collaborate with participants to secure vital documents, connect them to mainstream benefits, and support life skills development using Housing First, harm reduction, and trauma-informed care approaches. Provide crisis intervention and facilitate wellness groups to promote housing retention and permanent housing outcomes.
Required Qualifications
- High School Diploma or GED or Equivalent
- Valid CA Driver's license
- reliable car

- an acceptable driving record
- Willing and able to transport Clients in your personal vehicle
- Experience working with participants with mental illness
- Experience working with participants with chronic health issues
- Experience working with participants with substance abuse disorders
- Experience with Outreach and engagement
- Experience with crisis intervention
- Experience with suicide assessment/prevention
- Experience with affordable housing
- Experience with housing rights
- Excellent human relations/communication skills
- ability to act in professional manner regardless of circumstances
- Detail oriented
- excellent time management skills
- Strong verbal and written communication skills
- knowledge of Microsoft Suite (Word, Excel, Outlook etc.)
- Ability to self-motivate

Desired Qualifications
- Bachelor's degree
- a minimum two (2) years' experience working with homeless individuals
- preferably those living with mental illness and/or substance addictions
- an equivalent combination of education and experience
- Minimum of one (1) year experience in case management
- Knowledge of Housing First
- harm reduction
- trauma-informed care principles
- Experience using HMIS or similar database
- Strong documentation and organizational skills
- Ability to work effectively with diverse and high-acuity populations
