Job Summary
Case Manager responsible for coordinating and supervising clinical care for residents in a senior living/ adult care setting. Responsibilities include ensuring adherence to the resident Bill of Rights, communicating with residents’ care providers, coordinating admissions and re-admissions from community, nursing homes, and hospitals, maintaining clear documentation, and collaborating with a multidisciplinary team to support resident satisfaction.
Required Qualifications
- Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
- Excellent phone skills and ability to work with families/other care providers
- Strong organizational skills
- Ability to work cooperatively with multidisciplinary team
