Job Summary
Conduct intakes for clients referred to the Emergency & Practical Assistance Services Program and provide support, advocacy, and crisis intervention to veterans and their families. Devise individualized service plans, connect clients to community resources for housing stability, and assist with life skills training and budgeting. Perform regular home visits, conduct housing inspections, and verify tenancy with landlords. Ensure accurate completion of service information and financial paperwork, entering client data into HMIS and conducting quarterly progress reviews. Actively participate in case conferences, staff meetings, and supervision sessions. This full-time role in Syracuse supports veterans in Onondaga County through the Salvation Army's Veteran Case Management program.
Required Qualifications
- Specific experience in mental health services
- Knowledge of community resources

- Reliable private transportation
- Valid NYS Driver License
- Background and drug screens

- Bachelor's degree in human services or related field
