Job Summary
Case Manager role focused on intake and assessment for people experiencing homelessness, conducting interviews and triage based on acuity, providing information, referral and advocacy on accommodation options (emergency/crisis, short-to-medium term, private rental), identifying risk factors and escalating to senior leadership as needed, applying the Homelessness Model of Care in case management, maintaining and recording assessment data, and collaborating with TSA staff and external services to support clients. Fixed-term, 24 hours per week, located in Port Augusta, salary/conditions aligned with SCHADS Level 4. Strong emphasis on compassionate, inclusive engagement, data management, and building positive client outcomes within a welfare/social services context.
Required Qualifications
- Minimum Certificate or Diploma in Social Welfare or related discipline
