Job Summary
The Case Manager is responsible for managing a caseload of up to 25 participants in a recovery-focused transitional housing program. Key responsibilities include conducting assessments, developing individualized action plans, providing referrals, and facilitating crisis intervention. Candidates must possess strong verbal and written communication skills, empathy, cultural competence, and organizational abilities. A high school diploma is required, with college coursework in social services or psychology preferred.
Required Qualifications
- High school diploma
- Strong verbal and written communication skills
- Ability to express empathy and foster collaborative alliance with clients
- Familiarity with San Francisco social services, government resources, community resources, healthcare services, mental health support and housing services
- Excellent organizational skills
- Culturally competent and able to work with a highly diverse population
- Excellent time management skills
Desired Qualifications
- Valid CA Driver License and completion of organization’s driver safety training within introductory period
- Bilingual in Spanish preferred
- Knowledge of co-occurring disorders and trauma informed environment
- Strong electronic communication skills, e.g., Outlook
- Proficient in Microsoft Word & Excel
