Job Summary
Act as central point of contact for Participants in Kentucky Waiver Programs; create and manage personalized care plans; build relationships with participants, families, and service providers; complete required training (trauma-informed care, abuse/neglect reporting, professional boundaries, person-centered thinking, incident reporting); engage in local travel ~50% to conduct in-person visits and attend appointments.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, or related field
- Bachelor's degree in any field, plus at least one year of Human Services experience
- Associate degree in Behavioral or Social Science field, with two years of Human Services experience
- Kentucky-licensed RN with two or more years in aging or disabilities
- Master’s degree in Health or Human Services field
