Job Summary
Conduct intensive case management for approximately 20 high-acuity clients, focusing on housing stability, self-sufficiency, and community integration. Assess barriers to housing placement, implement interventions for recently housed clients, and teach life skills such as budgeting and meal preparation. Collaborate with LA County DMH and DHS to coordinate treatment teams, complete biopsychosocial assessments, and maintain accurate progress notes and physical charts. Transport clients in your personal vehicle to healthcare providers and agencies, assist with benefit applications, and facilitate access to primary and mental health services. Work Monday through Friday, 8:00am to 4:30pm, onsite at The Mayer in Los Angeles.
Required Qualifications
- High School Diploma or GED or Equivalent
- Valid CA Driver's license
- reliable car
- auto insurance
- an acceptable driving record
- Willing and able to transport Clients in your personal vehicle
- Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers
Desired Qualifications
- Bachelor's degree
- a minimum two (2) years' experience working with homeless individuals
- preferably those living with mental illness and/or substance addictions
- an equivalent combination of education and experience
- Demonstrated knowledge of case management
